| Comments: I like the check system. It makes it easier and faster for the admitting nurse who, in some situations, may be the only RN on that particular unit. |

| Comments: Finally, a nursing assessment that assesses nursing and not medical. Most forms tend to guide the nurse in collecting medical history instead of nursing. This form clearly outlines what the nursing needs of the patient are. Excellent |

| Comments: Finally, a document that addresses nursing assessment and isn't laden with medical information. The physician's H&P should have detailed medical data, not a nursing assessment. Nursing care is not addressed when the focus of the assessment is medical and not nursing. This document guides nursing care and is a nice supplement to the physician's H&P. |
